      Skilled Nursing Care

Home health nursing is the most common home health service doctors order for their patients. Skilled nursing can include care for both elderly patients and younger patients. In-home nursing care can help you recover from an illness, surgery or injury. Fraternity home health nurses can also help you manage a chronic illness such as COPD, diabetes or heart failure. They bring the same caring, compassionate presence as a hospital nurse, but in the home setting.

Physical Therapy

Home health physical therapy may be part of your care plan if you are having difficulty moving around your home or have had falls due to pain, illness, injury or surgery. For example, in-home physical therapy may be prescribed for you by your doctor if you qualify for home health and you have the following:

  • Surgery like a knee or hip replacement

  • Recent heart attack

  • Acute-onset medical condition

  • Exacerbation of a chronic medical condition like  heart failure, COPD or diabetes

  • Trouble with balance or falls

  • Difficulty walking or moving around

  • Pain after an injury, surgery or illness

  • Functional decline related to a progressive neurological condition such as Parkinson’s, Multiple Sclerosis or ALS

  • Suffered from a stroke and have loss of use of one or more extremities

Our at-home physical therapists and physical therapy assistants help you move with greater ease in the comfort of your home. Your plan of care is based on your needs and your doctor’s orders. Physical therapy for seniors is designed to restore normal mobility or maintain the highest level of functioning possible, so you can do daily tasks and activities you enjoy.

Occupational Therapy
If you are eligible for home health and have an illness, injury or condition that makes it difficult to do everyday activities, Fraternity home health occupational therapy (OT) may help. Losing the ability to do things that used to be easy, such as putting on shoes, holding a pencil or making a meal can be a frustrating part of aging or rehabilitation. Occupational therapy for adults may help you regain a sense of independence, safety, choice and control in the comfort of home.
Speech Therapy
Home Health Aide
Certain illnesses, injuries and medical procedures can make it difficult to care for yourself at home. Through hands-on help with bathing, dressing and other personal care needs, Fraternity home health can help improve your quality of life and keep you safe at home.

Speech therapy at home can help eligible patients improve essential skills such as swallowing, processing information and communicating. Often, home health speech therapy patients are dealing with medical issues like stroke, dementia, cancer or brain injuries. Others may have speech issues related to changes in the vocal cords and muscles that often come with age.

If you’re struggling with any of these issues and are eligible for home health, there is no need to suffer in silence. Fraternity Home Health speech therapists can work with you, your caregivers and your doctor to create a treatment plan tailored to your needs. You’ll receive encouragement, support and tools to help you manage your condition, in the safety and comfort of home.
